No Provider-Patient Relationship

Accessing the website, joining a mailing list or waitlist, downloading material, submitting a general inquiry, or communicating through a public website feature does not create a provider-patient relationship with Hollimere Health PLLC or Holly Pate, APRN, PMHNP-BC. A provider-patient relationship begins only after Hollimere Health accepts the individual for care and required intake, consent, and service arrangements are completed.

No Emergency or Crisis Services

Do not use the website, email, social media, or general contact forms for emergencies, urgent clinical needs, or confidential patient communications. For an emergency, call 911 or go to the nearest emergency department. Call or text 988 for the Suicide & Crisis Lifeline. Established patients should use the secure patient portal for routine, non-urgent clinical communication.

Website Submissions and Privacy

Do not submit detailed medical information through public website forms unless the page expressly identifies a secure method intended for that purpose. Hollimere Health’s Website Privacy Policy describes information practices for website users. The HIPAA Notice of Privacy Practices addresses protected health information maintained by the practice in its role as a health care provider.
